FOOD ALLERGY/NON-FOOD ALLERGY FORMS CTC 2017

PARTICIPATION BY A CADET WITH FOOD ALLERGIES

Recent events and experience indicate that the previously used parental waiver form "Participation by a Cadet with Food Allergies" regarding cadets with food sensitivities, does not fully comply with medical and legal realities of today's society.

D Med Pol has provided new forms that must be filled out for individuals who have identified anaphylaxis allergies and/or food sensitivities. Attached is a cover letter for parents, signed by the Comd, regarding the reasons behind the new forms, as well as the forms themselves.

  • The Anaphylaxis Consent Form as to be filled out by any cadet who is required to carry an epi-pen.
  • The Participation by a Cadet with Food Sensitivity form as to be filled out by any cadet who has a food sensitivity (food intolerance, chemical sensitivity, food allergy, life threatening food allergy (anaphylaxis). If the form is signed due to anaphylactic reaction to some food, the Anaphylaxis Emergency Plan must be filled out.
  • The Anaphylaxis Emergency Plan as to be filled out for any cadet who has one or both of the forms filled out listed above. For the 2017 CTC season, a signature by a physician is not required, nor is a photo.

The health and well-being of cadets is always our highest priority. As cadets prepare to take part in summer training activities, where they will be "in-residence" at our training centres, it is essential to both the medical staff and CTC staff to have additional information with regards to anaphylactic allergies.

All applicable forms must be signed by the parents of cadets heading to CTC this summer. Acknowledging that summer training is coming quickly, for this year only, the requirement for a photo and physician signature on the Anaphylaxis Emergency Plan form will NOT be required. This is a waiver for summer 2017 only. Since these forms are required for CTC this summer, priority should be placed on affected cadets who have a confirmed course or staff cadet job.

Cadets must bring these documents to the Cadet Training Centre, where they will discuss their condition with medical staff when they arrive. The medical staff, in conjunction with course staff, will be able to provide a recommendation to the Commanding Officer about the ability or inability of the training centre to accommodate the cadet.

At the CTC level, every effort will be made to ensure a cadet is not sent home because the forms are not filled out; it will be the RCSU CO's decision regarding any RTUs as a result of incomplete forms.

There is a clarification that needs to be made with regards to the documents attached. Medic alert bracelets must be worn if owned (and safe to do so according to the activity).  Cadets are not required to procure a medic alert bracelet in order to take part in cadet activities.

Staff, parents, or cadets who have questions about theses forms, should contact the RCMLO at: 1-877-381-6857.

Recent events and experience indicate that the previously used parental waiver form "Participation by a Cadet with Non-Food Related Anaphylaxis" regarding cadets with life-threatening allergies, does not fully comply with medical and legal realities of today's society.

D Med Pol has provided new forms that must be filled out for individuals who have identified non-food related anaphylaxis allergies. Attached is a cover letter for parents, signed by the Comd, regarding the reasons behind the new forms, as well as the forms themselves.

  • The Anaphylaxis Consent Form is to be filled out by any cadet who is required to carry an epi-pen.
